Health affected my undergrad and I want to go to Graduate School

My situation in short is that I have a couple physical problems (that are finally being treated) and PTSD. During my first couple years I didn't do very well because of the problems and constant doctor appointments taking away from my time. I have a 3.0 even now and will be going part-time this fall/spring to finish up. I couldn't be involved in anything because of what I mentioned. (I seriously should've been on disability)
I want to add that I have two WithdrawPassing's on my transcript and have mainly B+'s A-'s in my 400 levels (in the past year), but I took a 120 math last summer and failed (Not to make excuses, but I guess I am. I was given the wrong date to withdraw and had a seizure the evening before the final) How much is that going to affect everything?

I want to go for a degree in Arts Management and my undergrad is in Sociology. Now that everything health wise is really being taken care of I'm trying to get an internship and some volunteer experience to help me get accepted. I'm wondering if this will be enough and if it isn't what else can I do? Also should I mention my situation in my addmission essay and should I mention it when I talk to someone in the department informally (for more information). I have my heart set on this program at a couple schools and feel like I can handle it compared to my undergraduate years. Are there any tips that anyone can share?I appreciate any help and insite. Thanks!
