My guidance counselor gave me a sheet of questions to fill out before she writes my recommendation- one of the questions is 'are there any external factors that affected your grades I should know about?'. Sophomore year I was depressed, but I was never diagnosed by an actual doctor. Soph year seriously affected my grades though. Should I put depression down as a reason my grades were bad?

Don’t use it as an excuse, but let her know why it happened. She may mention it, but it will not be enough to compensate for the bad grades.
