As the title states, One of my teachers for my letter of recommendation does not do online recommendations. Where can I find a hard copy teacher letter of recommendation so that it can be mailed? Also, where is the address to which the recommendation should be mailed to? Thanks! :)

As far as I know, and I am no expert on this, but its been 4 days and nobody has replied. You will need to download the coversheet from your MyMIT account and attach it to any document you submit. Make your that your name and date of birth is on the form. As far a I know, the address for mailing materials to MIT is:

MIT Undergraduate Admissions Processing Center
P.O. Box 404
Randolph, MA 02368

I also believe this is the case – mail used to be sent to the admissions office on MIT’s campus, but it seems that the directions on the website now lead to the processing center.

But the only cover sheet on the MyMIT account is a supplemental recommendation sheet. If this isn’t a supplemental recommendation, what do we do?

The purpose of the coversheet is to ensure that the document gets correctly filed into your application. That is the most important thing in a case like this one. I do not want to tell you what to do, but you might wish to consider using the official cover sheet, but altering it, or noting on it that it is not being used for a supplemental LoR.
