<p>The instructions are very clear that everything you submit (school rec, teacher recs and supplemental recs) must have the MIT cover sheet attached. It automatically prints your name and application number. One is definitely for the math/science teacher, another for the humanities teacher, another for the school and another for supplemental recs.</p>
<p>As I tell my math/science kid, reading is fundamental, so read the instructions.</p>

Sending Additional Documents</p>
<p>There is absolutely no expectation that you submit additional documents beyond the required application components. However, if there are other pieces of information or other recommendations that you feel would add important information to your application, you must download and print an MIT Supplemental Document Cover Sheet and attach it to any non-required document you submit.</p>
<p>ALL SUPPLEMENTAL DOCUMENTS MUST HAVE A COVER SHEET ATTACHED.</p>
<p>To me it seems as if only supplemental information (i.e. not teacher or counselor recommendations) require a cover sheet.</p>

If your teacher or school has their own forms or just write a letter with no form, please print the appropriate form(s)
above and ask them to attach the MIT form to the front of their evaluation/report. </p>
<p>The forms themselves have your unique application number automatically entered. So, if you want to be sure that your recommendations stay with your application, I’d say use the forms. Why risk it?</p>
