<p>If I recall correctly all of that information can be found on the evaluation forms.</p>
<p>Teachers generally fill out the evaluation form, then write a separate letter of recommendation and attach it to the evaluation form. They send the form and letter via postal mail to the admissions office at the address on the form. (It’s also possible for them to submit evaluations electronically, if your school participates in Naviance.)</p>

<p>They can, but it’s preferred to have the evaluation form as well. But if they address the items on the form in the letter, the letter alone can be sent.</p>
<p>Mollie I live in Central Asia and I doubt if I send recommendations via postal mail they will be delivered on time, or even will be delivered.</p>

<p>Well, by “postal mail” I’m also including courier services, not just the national post.</p>
<p>The letters can also be submitted by fax, if that’s more convenient.</p>
