<p>I can't seem to find a downloadable health form on that site.
Also, how is this different from the Wellesley College Health History and Medical Evaluation form that they stuck in the middle of the white packet?</p>

<p>My daughter did not receive a Wellesley College Health HIstory and Medical Evaluation Form in the white packet. Wonder why not - "collating team malfunction"? Now, June 5, I notice this posting - well, I guess it's better than on June 20th.</p>

<p>Yep, I called them immediately and found out what she was missing. She was missing all of the health forms, roommate info, final transcript and etc. All members of the class of 2010-- check and make sure that you have all of the forms listed on the master sheet of forms and due dates - which was also missing from my daughter's packet.</p>
