<p>Yes, it is required. I can't recall where the health history form is, but I know that I downloaded it. You can always call the health center at NYU (it's the same place where you send the immunization form) and ask them where to find it.</p>

<p>guys you dont have to worry about getting everything electronically, i just called again today and they said all of the forms (immunizations and health history) will be mailed to us in mid-july and they arent due till beginning of sept. (i assume when classes start?)</p>
