<p>Ok, so this might be a stupid question but could someone tell me something about immunization/health records and stuff</p>
<p>someone said they were suppose to be sending us information about that in the mail and they haven't yet</p>
<p>i just returned from orientation and they didnt mention anything and the only thing that i came across were a some forms that were lain on the table on the side, you know the ones that give you free pens and free things...and those tables were completely optional, so i doubt thats the way they're going about telling students things, especially things as important as immunization...</p>
<p>so whats up. our their forms online that i need to download? what's required? assuming that i have no idea where to start, where do i go?</p>
<p>I think it's safe to obtain a record of your immunizations and make sure they're updated. Eventually they'll send the forms, and you can fill them out and whatnot. I tried looking for the forms online, but for incoming freshman, I think they send us the forms by mail only?</p>
<p>Fill them out, then mail or hand deliver them to the Health Center. By the way, the Health Center is a gorgeous facility. We visited it when our daughter attended the CAS orientation last month, and we were really impressed.</p>
<p>These are definitely the correct forms for freshmen; we handed them to the proper people at the Health Center.</p>
<p>I also found these forms online a few weeks ago (after calling the health center) because my son was going to Europe and needed a physical, so I thought it made sense to complete the NYU medical forms at the same time. However, our pediatrician did not have the meningitis vaccine in stock and it was ordered and should be in next week.</p>
<p>I find it pretty shocking that NYU hasn't sent these forms, and that we parents are the ones finding out this information. School starts in only 6 weeks!</p>

I'was just wondering if the Health Center was giving tours during orientation, because we have tried to visit in the past on several occasions and were not permitted by the security guard who sits at the desk at the entrance to the building where the Health Center is housed. We had been in touch via phone to the Pharmacy for the past four years, and on graduation day, we tried to visit just to thank them in person for all that they've done, and were stopped dead in our tracks. This time, however, I immediately went outside the building and called the Pharmacy to tell them what happened, and they said that I should phone ahead next time, and they would call down to the guard and then she would let us go up. Since the Health Center is not in an NYU owned building, it's more difficult to get past security.</p>

<p>i actually got the stuff from nyu like...4 hours after i posted that haha. what are the odds!</p>
<p>and just a note, what they sent me to fill out and send in was not those forms at all. freshman apparently are supposed to fill out a full questionnaire of medical history. the packet they sent me looked a lot like a form for a standardized test, like the sats or something.</p>
<p>scrapnel: That's interesting--thanks for the info. I guess we'll look for that medical history form in the mail. I think the other two are required, though (the links I posted).</p>
<p>I think that the medical forms aren't due until September 30. I'll check when I get home from work and if the deadline is different, I'll let you know.</p>
