<p>So my new college (FSU) requires me to fill out some health immunization forms in order for me to be alligible to attend classes. Some of these forms need to describe my shots history and have to be signed by a clinician. I moved to the United States in 2011 and I have one document displaying my whole shots history. The doctors I went to so far refuse to sign my forms because I haven't had the shots taken at their clinic. How am I supposed to submit these forms?</p>
<p>It depends on how the form is set up. You might be able to just send in your medical records? I would call your school and ask for help.</p>

<p>That is the strangest thing I’ve ever heard - medical records are transferred from one doctor to another all the time, and the current physician should always be able to fill out a patient’s history based on those documents. No responsible practitioner would ever insist on re-administering all of your vaccinations just to fill out a form!</p>
<p>The only thing I can think of is that perhaps the document you have isn’t signed by a physician in your home country, or doesn’t provide enough detail for another physician to be able to rely on it? But if that’s the only documentation available to you, I’d agree - contact your school and ask for help.</p>
