<p>you just need to get a phsyical and have the doctor fill out when you received all of your shots. i think the doctor does sections 4&5 but i could be wrong.</p>
<p>yup that's it. However, if you are from the midwest you need a mumps vaccination, again.</p>
<p>My son went to the dr the other day. He had gotten the meningitis vaccine 2 years ago that was good for 5 years. Turns out there's a "new & improved" vaccine that is in short supply (at least in our region). that Dr advised he get. Turns out he got the last one they had & the 3 kids in the waiting room had to wait until the allotment for next month arrived.</p>
<p>I'm not playing Dr here & giving advise on whether or not you should get the vaccine. My suggestion is to call the Dr before your appointment to see if you need the vaccine & if they have it so you don't have to go twice.</p>

<p>they're online however you can't really go to them by a link. go to newstudents.cornell.edu and then go to Forms and then the link to Health History Forms. you have to do part of it online and then the server saves it as a .pdf with your info on it and then you have to print it out and bring it to your doctor. after your doctor fills it out you can send it to Cornell.</p>
