1) No, they look at the IBs just like they look to the APs by the Americans...they will see if it is good enough, see your GPA, SATs and that will all add up to become your 'academic profile' for them.

2) The predicted will only get you so far as the acception. Once you are accepted you HAVE to send in your FINAL IB scores when you receive them, and at that point they may wish to remove your acceptance (or perhaps give merit scholarships).

3) If top you mean Ivy League you are thinking of 42+ to have a good shot or at least over 38/39 to have a shot. For any other good one (like JHU) about a 36+ should do it.

4) Not sure about this one myself, but I am gonna say yes, even if it is estimated.

5) You mean what universities in the uk and us? or schools? If Universities you are talking about Cambridge, Oxford (38-42+), Edinburgh, Glasgow, Manchester (35+), LSE, Imperial, UCL (38+) and more.

6) They do not count usually for American universities, but for UK they do. Since they will only be receiving your predicted IB grades they want to see something concrete on your academic achievements, therefore your IGCSE scores.
