I'm rather confused with the briefly describe portions of the Activities & Award section. I'm not sure of what to put.

It says:
1. Add and briefly describe any unpaid volunteer work and community service you have performed beginning in ninth grade. Note any leadership positions you have held.

My questions:
-In the organization name, do I just put the name of the organization (including school community service clubs) or do I put, for example, "secretary of Key Club"?
- Do I describe what the organization/club is? Or do I describe what I did while in the club?
*Can you give me examples?
- I only know the total of my service hours, so I'm not quite sure of what to enter for the "Hours per Week" and "Weeks per Year" section. Do I just estimate?

Add and briefly describe any extracurricular activities in which you have participated beginning in ninth grade.
Examples: team captain of varsity soccer team, student body president, school yearbook editor, first chair of high school symphony orchestra, lead actor of high school play, JROTC Leadership Education Training.

My questions:
- Once again, do I just put the name of the activity or do I put my leadership positions?
- I don't know how to describe the sports I've participated in! I'm sure they don't expect me to describe the game play. Could anyone possibly give me ideas? I've been in the Varsity swimming team and the girls' waterpolo team from 9th- 12th grade. In regards to waterpolo, I was in JV from 9th to 10th grade. However, in 10th grade, I played in both Varsity and JV. From 11th to 12th grade, I was in Varsity.
- How would you briefly describe Color/ Winter Guard?
*Can you give me examples of what you would put for your extracurriculars/sports descriptions?

Other questions:
- Do UCs commend the Honor Roll award? At my school, it's a rather commonplace sort of award because so many students receive it, but I was just wondering if the UCs acknowledge it.

Thank you for taking time to look over my woes! Help is very much appreciated. :)
