How can I apologize to my teacher?

<p>I have been having a lot of AP US History issues this year. My class is first period and I am late to class about 75% of the time. I am not purposefully late; I have to commute to school for an hour each morning on the train, and I often get caught up. I try to leave earlier but I am still late most of the time. I can't excuse my actions, but I feel really bad about it. Also, lately I have been very overwhelmed with school--some personal, stress/anxiety issues--and my homework hasn't been up to par lately. I honestly feel really terrible. She is a good teacher, and I could've been a much better student. I participate in class often and I try to contribute.</p>

<p>My grade has gone down (over the course of the year I've gone from a 95 to an 86) and I would like some extra credit. I don't want to be misunderstood, and make her think the only reason I am apologizing is to get extra credit or boost my grade. I do genuinely feel bad. </p>

<p>It's also tough because it's the end of the year, and I feel like it's too late to make a significant change in her (entirely justified) bad impression of me. How can I make amends? </p>
