How do colleges and/or scholarships calculate unweighted GPA?

Is it the old standard:

100 = 4.0
99 = 3.9
98 = 3.8
97 = 3.7
.
.
.
90 = 3.0
89 = 2.9
88 = 2.8

Do they include every class such as PE, Band, Office Aide, and etc.? Yes, I've done a search for the answer but it's usually been vague so I'm still unclear.

In general, do colleges calculate by each six weeks, by each semester, or by the final grade each year?

Depending on how it's calculated, it might give the extra few points to be eligible for specific scholarships or getting into the Honors program. How many decimals do they usually want and do they round up? If you have a 3.45, does that usually round up to a 3.5?

Also, when applying for scholarships before HS graduation do they usually want the previous full years' grades or would they also want the portion of your Senior year's grades (i.e. say the first few six weeks or whatever) as well?
