<p>It depends on if you want to do a page or a letter.</p>
<p>For a letter, I would center the information (in the middle of the page) and put, for instance:</p>

Anywhere High School</p>
<p>If you want to do a letter write something like:</p>
<p>To Whom It May Concern,</p>
<p>My name is John Smith (a current senior at Anywhere High School). I have enclosed a hard copy of my admissions essay for your review. </p>

<p>P.S. - If you have any comments, questions, or concerns feel free to contact me at _________.</p>

<p>You should also put a small header on each page, with your Name, SSN# and the type of essay listed.</p>

<p>I think EPTB means short answer or commonapp essay (maybe the topic as well).</p>
<p>I meant what the essay is-- meaning, whether it’s the CA essay, short answer or other supplemental essay.</p>
