<p>Mine was missing and they told me to send it by fax. It got there but page 2 did not come through and they emailed me to resend. I think that will work best for you. When is the deadline? If today they will know you tried even if theydon’t get it all Good luck!</p>
<p>i would recommend faxing because you can get a quick confirmation of whether or not they got it.</p>
