there are always a******* in this world who really don't really care about their students at all. Don't worry, one of my teachers is screwing up my grades (think whole class got no better than a B)

This is exactly why my school gives us official copies of our transcripts from the same computer that is used for the college ones. (In official envelopes and stamps that say "not official if seal is broken" and all that jazz too.) This way, we can confirm everything is right before senior year even begins.

op is at UCSD I think

A very similar thing happened to me (years ago) except it was when I was applying the law school and the LSDAS people sent someone else's grades to the schools I applied to instead of mine (unfortunately, it wasn't someone with a 4.0 at Harvard!) By the time we figured out what happened (I couldn't understand why I was rejected from my very safe school) it was too late - I ended up waiting an extra year to start law school. (In the long run, not a tragedy but VERY aggravating at the time.)

Anyway . . . there's a valuable lesson in OP's story - ALWAYS check your transcript for accuracy. Always check your SAT/ACT reports for accuracy. Always double check your mid-term grades, letters of rec, etc. have been sent. Even though it might be someone else's job to do these things, only you really care what happens to you. And only you will suffer the consequences of any mistakes.
