Immunization Form

<p>My doctor is going away for a 2 weeks.. and the form is due the June 15th.. Is it okay if the form is not submitted by June 15th.. i got an email if all forms are not recieved by June 21st my admission will be revoked.</p>

<p>That email said that you would get revoked if the community standards form wasn’t in by June 21st.</p>

<p>Wait what email?</p>

<p>They sent me a little threatening email because they didn’t get my community standards form (which got lost in the mail) that it needed to be in by June 21.</p>

<p>Thankfully they let me scan it and email it to them so I have it checked off. I think this guy is confusing the immunization forms with this. I have phoned them three times about this and they keep saying “we should have them by the first day of classes.”</p>

<p>^ The same exact thing happened to me… they lost my fax i guess and I had to email them the form.</p>

<p>Why I haven’t received any email requesting the immune form? I only on their website that “Students will be WITHDRAWN FROM THE UNIVERSITY 30 days after classes begin if immunization requirements have not been met and the Immunization and Medical History information have not been received by Campus Health Services staff.”</p>
