Immunization Requirements Form (Which??)

<p>I know thisll probably be a stupid question but right now I'm confused as to which form I should fill out. Theres a form for "Non-Healthcare" students and one for "Healthcare" students but I have no idea what that means.</p>

<p>Does non healthcare mean you dont have healthcare or does it mean youre not going to purchase the healthcare from NU or studying healthcare or what?</p>
