<p>I went to Orientation Part 1 in the beginning of July. I brought a vaccination/immunization record from my doctor that lists all of the vaccinations that I have gotten throughout the years. </p>
<p>The Nurse told me to fill out the Health Form and fax it over to the Student Health Services.</p>
<p>However now that I look at the form it says I need a stamp from my doctor/physician and it has to be signed by him.</p>
<p>However the stamp/signature is on the immunization/vaccination sheet that I have.</p>
<p>Would it be possible to stop by the Student health Services building before Orientation part II tommorrow and hand in whatever I have completed on the health form plus my own copy of the immunizations?</p>
<p>and if that does not work it says I will be deregistered from classes if I do not submit the requirements.</p>
<p>When would that take place exactly?</p>
<p>I know this is all my fault since I should have done this sooner instead of waiting to the last minute but I did not realize that the health form needed so much more since I have the vaccinations/immunizations all listed with dates however I do not know what goes with what.</p>
<p>I am going to call them tomorrow morning and ask them this same thing but I am just wondering as of now.</p>
<p>This is primarily for someone who has been in a similar situation like me in the past or currently.</p>
