<p>in the email NU sent out about health forms, they mention the Tuberculosis Self-Screening Form (<a href=“http://www.nuhs.northwestern.edu/evanston/tbselfscreen.pdf[/url]”>Northwestern Medicine Student Health Service Evanston - Northwestern University) and the Admissions Health Record (<a href=“Northwestern Medicine Student Health Service Evanston - Northwestern University”>Northwestern Medicine Student Health Service Evanston - Northwestern University), but page 3 of the Admissions Health Record is the TB self-screening form. Do we still need to submit both?</p>

<p>No penalty unless you don’t have the form in by 30 days after the start of classes OR if you still have an immunization deficiency at that point (ie, if you don’t have all the required immunizations)</p>

<p>No physical required, as far as I can tell…</p>

<p>Seems like they <em>want</em> to have the form in well before then, so they have time to check that students are in compliance, but there is no penalty until the above-mentioned date.</p>

<p>AND - if you submit a copy of your immunization records, you don’t have to fill out part II, so you don’t need a doctor’s signature (see the instructions at the top of the form).</p>
