I am planning to send in videos of myself playing my instruments. And I would like to know what kind of format or limit of the video if there is a preference.

I don't know that info, but are you applying to Peabody also? If so, i bet they would already understand your musically inclined. I didn't apply to Peabody, but made sure to highlight my music stuff in the resume and my essay. I'm sure your supplement won't hurt and AdmissionsDaniel will have the details.

thank you fragaholic, I will just wait for AdmissionsDaniel for more info.

And do I have to apply to the Peabody in order to send instrument demonstration supplements? I just want to join ensembles in Johns Hopkins instead of majoring in music.

I have answered questions about supplements numerous times. Almost all the answers can be found here:
Hopkins Forums -> Supplementary Material w/ Application

We do not provide any guidelines about arts supplements. Use what ever format you want.

DO NOTE:
If you are a Fall 2008 applicant and you are submitting your supplement now, THERE IS A STRONG POSSIBILITY it will not be reviewed. Supplemental information was supposed to be submitted by the application deadline. There is no guarantee that any items added now to one's application (aside from mid-year reports) will be added to one's file in time for review.
