I completed a phone interview a couple of weeks ago and since the school is my top choice, I'm wondering if it's worth it if I send them a letter telling them that they are my first choice. Does it make a difference or at worse, will it make me look like I'm a bit desperate/not confident enough in how things went?

You should totally send them an email saying they’re your top choice. Schools play a tricky balancing act extending enough offers to hit their target but not too many that would result in over enrollment. Telling a school you’re a guaranteed admit is information they do want. It may not be a huge push, but if it’s between you and someone else as to who goes on the waitlist and who gets accepted in the first round, it might make the difference.

I told the adcom chair of my program last year that they were my top choice and he emailed me right after the adcom met to tell me I’d been accepted.
