Letter of Rec Confusion

<p>In the common app instructor evaluation form, at the bottom where it says "to the instructor", there is a line for "College or University" and the a line for "School Address"</p>

<p>Are you supposed to write your current school here? Or, do you write the school you are applying to?</p>

<p>Okay, thanks. I was just confused because at the top of the same form, it tells you to list the “College or university you now attend” but then at the bottom it says “College of University”</p>

<p>I assume we can make copies of these forms, right? As in, there doesn’t need to be an original signature for the waiver for every college we apply to.</p>

<p>That may be in reference for the recommender, then. I’m not sure. </p>

<p>If you print the form out it only takes two minutes to fill it out two times.</p>
