I got a volunteer position at a hospital and my boss said that she would write me a letter of recommendation for college. How do I do this- Do I get an envelope with my school stamp on it and then once the letter is written give it to my guidance counselor?
+ with teachers, if I want a letter of recommendation from them, do I have the school give me an envelope, and once they're done, do they mail it to the college? Does that mean if I'm applying to 6 schools my teachers would have to send 1 / school?

It depends on how your high school handles recommendations. Some schools keep the teacher recommendations and mail them in with the counselor recommendation; other schools allow you to mail them separately. Usually, you give your reviewer an addressed, stamped envelope. (S)he writes the recommendation and signs his name along the fold; after that, (s)he either mails it or returns it to you to mail with the rest of your application.

Since the rec isn't from one of your teachers, you should probably be able to have your boss mail the rec directly to the college. Just give the teacher an addressed, stamped envelope and have her mail in the letter
