I have 91.6 ~ 92% in my Academics and I'm from India, I am second in the class of 100 students. How will this effect my admissions in US universities?

Is the class rank considered or the percentage?

Usually most colleges and universities will try to put your school system in context to know your academic performance compared to your peers. Since most top colleges frequently get applicants from overseas, they will probably understand your situation.

Your school profile will need to make VERY CLEAR how good that 91-92 is. While most American colleges know international systems, they may not know precisely (it’s impossible to have precise knowledge of ALL systems :p) Not only should that score be compared to your school, but if possible there should be an indication of what it means in relation to the province (ex: “it means s/he’s # 2 in his/her class and in the top 10% for the province”, or whatever may apply).
Each grade should have an indication such as: 5/500 typically get that grade; 50/500 typically get that grade; 100/500 typically get that grade)
and of course whether your school has cut offs, recruitment scores for admission, etc
