but I got a different one from you guys, I got the "Medical Condition/Disability" one.
I submit it, send in the letter of recommendation.</p>

<p>1.) is this a smaller group of people who get this questionnaire?
2.) Around how many overall students get supplemental?
3.) has anyone gotten in with this type of questionnaire?</p>
<p>and 4.) did anyone else received this type of questionnaires?</p>
