I know this may be such a newbie question but I have no idea what to do with my medical forms. They require dates of my immunizations yet… I don’t know where to get this information. From the high school or what?

<p>I guess the first question is why don't you have a doctor... did you used to have one? Are you just between doctors? Are you progressive and anti-doctor? An old doctor may still have your files. If this is an option give them a call.</p>
<p>Where did you get your immunizations? The hospital/clinic/doctor/office may be able to give you a record.</p>
<p>If you're unable to find the info through one of those sources, your high school's nurses office may be able to give you the information as well. I don't know specifics about your HS, but we had to fill out a medical form with our immunizations every year, so it's very possible they'll be able to feed that information back to you.</p>
<p>In the future always keep track of your immunizations. My pediatrician gave my mom a card to keep track of my shots when I was born and I have all my immunizations records for the past 20 years in my nightstand. If you visit a doctors office or hospital they may be able to give you something similar.</p>
<p>I just .. don't. I've never needed to go to the hospital EVER in my life except for maybe mandatory checkups or whatnot. Sorry but the expense of one isn't exactly what my family can afford.</p>
<p>I'll get the immunizations from the nurse at my school and probably just get a physical or something done .. I think, if that's okay with the whole procedure</p>
