Question about Health form

<p>I don't live close to my doctor at all, anymore.Is there any other way to get proof that I've been vaccinated for those diseases?Like maybe get proof from my school i guess...they should have proof,right?</p>

<p>If you go to your assigned guidance counselor at school, he/she will have access to your file containing a blue immunization form that lists all of your vaccines and when they were administered. Get a photocopy of that form and take it to the health department. They should be willing to fill in the medical/authorized personnel section (free of charge).</p>

<p>Actually, I believe your school can both provide a copy and sign it. Many schools have their data computerized, and some will find that it was on the transcript!!! Yup. Our school generates a 6-7 page computerized transcript that includes all grades, all SAT and ACT info, all AP tests and results, and the info from the Blue card. I would not have believed it if I had not asked for a copy of it to review last year before beginning the application process. I do not agree with the sharing of SAT/ACT/AP info (and neither does the college board) but also found out that many schools accept the scores this way without paying again to have them sent. </p>

<p>If you attach a copy of your schools immunization form, and they sign it and stamp it with the school stamp, they will accept it. It says "medical or authorized signature" and a the school is authorized to keep a record and copy that record.</p>
