Medical records and college admissions?

Hello,
I am a junior but I am posting in this forum because I've heard junior year is the most crucial and I am very worried about not meeting expectations.

Basically, as the title says, I have had some health problems that have caused some disruption from school. In freshman year to now I've been struggling with depression, though it seems better now. This past summer I had mono and the fatigue lingered for the first three months of school.

And currently, I have a D in AB Calc and a C in AP Physics C, but other than that, all As. The AB Calc is because the make-up policy didn't allow me to retake a test even though it was an excused absence. For Physics, which I have after lunch, I fell asleep a few times at the beginning of the year due to the lingering mono symptoms.

I may, at present, be coming down with some other form of mono or something again. My lymph nodes are swollen and it really feels like it did when I had mono in the summer.

It is very weird because before high school, I never, ever got sick. But since high school started, I've been getting majorly sick every year at least once.

How would I inform colleges about this? And have I totally screwed my chances? And, what about summer programs? I really, really, really want to go to a research summer program this summer and hopefully be able to go on to further competition level for next year Siemens.

I would like to go somewhere that isn't quite ivy-level but is still kind of competitive, for example Bryn Mawr, Brandeis or U Rochester. Basically, a suburban campus is nice, and while I do not really care where the school is, I would prefer not to be landlocked. Also, I am interested in majoring in neuroscience.

Thanks in advance.

Your guidance counselor can include some information about your medical history. You don’t need to write about it yourself.

But you may want to have a chat with your personal physical about your concerns. Maybe your HS is an absolute stewpot of germs, maybe you aren’t eating or sleeping properly, or maybe there is something not quite right with your immune system. This is worth investigating.

thanks
hopefully there’s nothing serious though
