<p>I lost my little national merit first choice college card a while ago. I didn't think it was a big deal; I thought I would just call them up.
But.... Every single phone number listed on their website doesn't work for me- I get the number has been disconnected or some such nonsense message.
So I decided to just write them a letter (and keep a copy) that has all the same info that the card had.
So my question is, What did the postcard for declaring a first-choice college ask for (name, college, etc.) ? Any help would be much appreciated.</p>
<p>Hmm...I don't remember very well because I mailed mine in a while ago. I think it asked for:
-Your name
-Name of new college
-City/state of new college
-Your semifinalist ID #
-Your selection unit #</p>

<p>Semifinalist ID #, Name (last, first, middle initial), Selection Unit #
Home Mailing address (number & street and/or PO Box), City, State & Zip</p>
<p>My First Choice College is:
College Name, City, State </p>
<p>I will live (check one) on campus/at home</p>
<p>Signature Date</p>
<p>I just happened to have it here in my office!</p>
