<p>Also when my teacher wrote my evaluation, se didn't address it to anyone. She just put teacher evaluation 2 at the top. Wrote the reference and then signed below. Can she do that or does it have to be addressed to someone in particular.</p>

<p>Yes, they can send school forms by mail. And the teacher doesn’t have to address your evaluation letter to anyone, but she does need to fill out the Common App evaluation form. Hope this helps.</p>

<p>First off, know that only the person you designated as your Counselor (not a Teacher) can get your personalized Secondary School Report online. That could be what’s giving you trouble – in which case, you just need to designate someone as your Counselor.</p>

<p>If you’re applying to a school (e.g., Stanford) that requires the form with your application id printed on it, the generic form won’t work for that school.
