<p>For the waiver form, all you need to do is go to myberkeleyapp. Click on the overnight host program event link; then when it takes you to that page, on the left hand side click the FAQ and the 2nd or 3rd question on the FAQ “Do I need to bring my waiver form?” is answered with a link to the actual waiver form. Click it, print it and bring it with you.</p>
