psat writing help

i took the practice psat test found in PR's 11 tests for the new sat today. was confused by a few things in the writing section:

When Ms. Ruiz (arrived) at the holiday sale, she realized (that) she (had left) her wallet at home (and must) go back to get it.

i put E, correct answer D.

Pilot carelessness, (rather than) equipment failure, (was) (responsible) for the (near disaster) at the local airport.

i put D (thought it should be "nearby disaster"), correct answer E

also, is "a group of people" singular or plural? does the same go for "a number of", "a council of", etc.?

thanks

Pilot carelessness, (rather than) equipment failure, (was) (responsible) for the (near disaster) at the local airport.
E is the correct answer. Near is not refering to location, but rather the accident, meaning the almost-accident
