On the activities section, does scientific research (mentored) count as “Work: Internship” or just Math/Science? Also, I have done research all 4 grades of high school, but at times it is only 2-3 hours/week and other times (like the week or weeks before a competition deadline) it gets to be like 40 hours/week (it was that high once, but usually it still gets to be around 30/week). So, what do I put for “hrs/week” category?

Regarding your first question, I guess it just depends on the involvement of the mentor. If you are the one who designed and carried out the research, I would put Math/Science. However, if it was his or her research and you were just helping out, I would put Work: Internship. I don't think it will matter much either way, though.

I would average the hours per week. If you feel that this is not sufficient, just clarify things in the space for additional information.
