I was academically disqualified from university for a year and wrote a readmission letter. Any feedback would be appreciated.

c. A plan of action you intend to follow should you be readmitted, including any support systems and strategies you will utilize.
After transferring to XXX university, two quarters led to my academic disqualification following a third quarter of dissatisfactory academic progress. I ran into these troubles primarily due to my poor understanding how I learn best, especially within difficult subjects, and my inability to seek help. In addition, I was critical of everyone other than myself and I could not embrace the mental and social discomfort when respectively practicing for exams and visiting a professor or TA for assistance. As such, I did not take responsibility for my failure and unfortunately, only took action when it was too late.
While away from school, I recognized that did not put in the work needed to succeed in my classes. I only had a surface-level of understanding for these subjects and lacked a deep knowledge that could be achieved via practice. To fix this situation, I enrolled in XXX online learning course, which is a six-week online course designed to help address flaws in learning. From this course, I gained a new perspective on improving my study habits and setting goals for my academic success. Concurrently, I have studied programming on my own and uploaded several projects on Github. Subsequently, I enrolled in XXX Community College and took three courses: Web Development, Introduction to Linux and SQL, where I received an A, A, and B+, respectively. Although I had trouble in my major, I would like to continue to study applied mathematics, as I remain very interested in the subject. I will retake the courses I failed and hopefully succeed with my improved study habits.
If I were to be readmitted, I plan to continue the medication plan prescribed to me by the psychiatrists, as recommended by XXX School Counseling and Psychology Service, and continue the treatment and services necessary to improve my academic performance. I will be careful in choosing my schedule and apply the skills I have previously learned. With the preparation I’ve made, I believe I can be successful in gaining good academic standing.
