Well for me, it may be different for others, I just went up to teachers that know me pretty well and asked them to write the letter of rec for me

Well ... I've never actually sent any recommendation letters for a scholarship (but I have a lazy older sibling that asked me to help her with her recs for college this year).

So, based on that information, I will outline what (I think) you should do:

1) Find your recommendation forms (they should be included in the Questbridge Application).

2) Look to find any instructions on the application or recommendation forms. (As in the address to which the recs are sent, etc.)

3) The "package" you will then make will include an envelope (I'd use a business envelope if no other size is required/asked for). Inside put the empty recommendation letter. Then on the outside, write the address of where Questbridge asks you to send the application/recommendations.

4) Check how much it costs to send an envelope of that size and then put the correct amount of stamps on the envelope.

5) Find the two teachers/counselors you want rec's from. This should be done (at least) two weeks from the due date of the application but it's better to leave 3-4 weeks in between when you give it to your teachers/counselors and when it's actually due (this way your teachers will have enough time to fill it out and mail it without stressing themselves out).

6) Choose teachers/counselors that like you and/or know you very well. Be sure to inform them of when the due date for the rec's is, so that they do not miss the deadline!

7) Good luck and I hope you get the scholarship!

Many teachers also ask for a resume of sorts, just mainly a page of things you do and that you're proud of, that way when they say afcartman is hard working they have a great example of it.

Before doing too much of anything I politely asked if the teacher would be comfortable writing a recommendation letter for me. I printed out a copy of my Academic Resume which I would deliver to the teacher along with an addressed and stamped envelope. If the school required a particular form, I of course provided that as well.

Googling resume would give you a good idea of how to format one. Many word processors have templates which you could use as well.

First of all, this QB scholarship thing does not require an envelope. It's all done online. All you need is to give your teacher the instruction thing on the site. He/she will then fill it out and submit online. I'm doing the same thing, just gave my teacher the instructions sheet today. Then I emailed him the due date, etc., just for more info. I'm scared that he'll write a bad rec though..oh well.
