<p>Three of my teachers have agreed to write me a letter of recommendation.
They have each wrote a letter. Later I saw there is a teacher evaluation form in the common application.
Is the recommendation supposed to be a letter or do they have to fill out the common application: teacher evaluation form?</p>

<p>They can just do the checkboxes, attach the letter, and say "see attached".</p>

<p>When the teacher makes copies of the letter, do they have to sign each one or can their signature be copied like the letter?
