Recommendation Resumes

Hello,

I've gotten alot of useful and very supportive information from this forum in the past and now it's that time again where I'm all over the place about school. I've been planning to get teacher recommendations for awhile now but, just me being me, I procrastinated all this time on it and now theres less than 15 days left until most college applications are due and I've yet to even have a school decided or an application form.

Well anyways, I decided I would start it all by first getting the teacher recommendations out of the way, and many people have told me that I must write up a resume. I've never written a resume before, even for work, and am totally dumbstruck about it. If you guys can please help me out on this one...

How exactly do I structure it?

What types of information should I include onto it?

(I was thinking maybe GPA, clubs I have participated in, sports I participate in, etc...)

Should I make a different resume for every teacher I have write one?

If you guys could please help me out here I would GREATLY appreciate it. I'm pretty embaressed to say that the internet is only place I have to talk to people directly about the whole college process and get help from... noone in my family has ever went through this stuff and so I have no guidance as to what to do to get where I want to get.

anyone? ... please

For your purposes, I am sure it doesn't have to adhere to a set form. Just include relevant information, detail your activities, awards, and anything else the recommender should know. Relax. It's not complicated.
