What colleges require them? Most of the schools that I'm applying to ask for teacher evaluations from 2 academic subject teachers. But is there a counselor's letter of rec. that is sent with the transcript or something? I'm somewhat confused, since my school's counseling office keeps on reminding us to ask for recommendations from teachers and counselors, but I'm not sure what it is required for. Should I request one from my counselor?

There is no one single answer. You have to start with the schools you're considering. Look at the undergrad admissions page of their website. Most require a Counselor's rec but some do not. Get the form off the school's site or off the Common App site if they use that and give that to the counselor.

On the common app, there is an evaluation section for the guidance counselor on the secondary school report form. Is this equivalent to a letter of rec and do I need to go ask for it?

Yes, that's the form for your counselor and there's another one for the teacher. Check with your guidance office, they probably want you to include an envelope with that form when you give it to the teacher and counselor. You'll need to address the envelope (to the address that the school says to send info to), stamp it and give it to your counselor and one or two to teachers (depending on if the school requires 1 or 2 recs). Then they mail it in.

The counselor form online is basically the secondary school report form right?
