Reminding Professors/Bosses of Rec Letters

I asked a former boss for a recommendation letter for graduate school about two months ago. I told him I needed the letter by September 24th, so two weeks ago, I e-mailed him about a trip I had taken (for some small talk) then reminded him of my rec letter and asked him to contact me when he was done. I also thanked him again, of course.

Well, I still haven't heard back from him. I really don't need the letters until November, but I purposely asked him early so I could make sure he got it done.

How should I go about asking him or reminding him? I don't talk to him very often, so I don't want it to seem like I'm only contacting him to get the letters (even though that's what I'm doing.

Could someone give me an example of what to say whether through e-mail or by calling him?

When it’s due in a couple of weeks, I go and visit in person about once a week. The week it is due, I visit every other day.
