<p>Put my name on the supplements? Because I didn't think of it until today, and I submitted it a week ago! Damn, I don't have my name OR birthday on any of the supplements...how bad is this? Will the college be able to keep track of my essay (like, will it be attached to the form part of the supplement)?</p>

<p>Also, I picked prompt # 5 but never actually wrote down the prompt that I picked. How bad is this?</p>

<p>Do you mean online or on paper?</p>

<p>If it’s online, CommonApp automatically puts your name and ID number on each page. Also, if you didn’t say prompt #5 but put your own prompt in, they should be able to get it.</p>

<p>Hopefully you didn’t do this offline . . . If you did, just print them off with your name/info, and mail them tomorrow. Postmark deadline is Nov. 2nd. </p>

<p>As for the question for #5, yeah, that may be a problem. Log into your UChicago account, go to “Update Your Application,” explain quickly what the problem is, and below paste the essay with the question you wrote for yourself preceding it.</p>

I answered a creative prompt that I came up with, but didn’t actually put the prompt itself on the essay and only answered the prompt but the adcom is going to think I’m crazy because they’re not going to know what I’m talking about?? I wrote “prompt # 5” on the top of the essay, but the prompt could be, like, anything… :[</p>
