school from is recommendation letter? HELP!!

I have no idea about the process of hand in the recommendation letter and transcript,
so I see some college ask us to hand in at least one recommendation letter from teacher and counseler, I thought I can just ask them to write me a letter and then I will give them the envelops and then it is done.

The problem is : when I ask a teacher to write a recommendation letter for me, it is fine, but the counselor who I asked her to write recommendation letter for me said that I have to print out the school form in Common application.

Is that mean the school form is recommendation letter.

So, the college want the school form or a recommendation letter?

By the way, if the school doesn't require recommendation letter, I don't need to mail them the recommendation letter from counselor, right?

I am so confused for about I used the school form......
By the way, if I choose to not use it for some schools, how so I show them my rank in class?

Hope someone can help me! thank you!!

To do this on paper (rather than electronically): go to the common app site but don’t log into your account. Go to the Download Forms section and look for Teacher Evaluation (this is the teacher recommendation form) and Secondary School Report (this is the counselor form).

All schools always require the Secondary School Report from your counselor. This is the form where class rank is reported.

Some schools recommend or require one or more recommendations (the Teacher Eval form) and some don’t.

If your teacher or counselor writes a rec on their own paper or a different form, that’s fine. They can attach whatever they wrote to the Common App form and submit that whole thing.

Thank you so much!
So, every schools need the school report,
this is a must, right?

Yes, every school that uses the common app needs the school report.
