<p>My school requires me to turn in our transcript request forms this Friday, but I just now realized that I never received the "school report form" that they were supposed to send you in the mail, around May. On the email it says: </p>
<p>"Finally, please remember that your admissions is contingent on our receiving your final transcript and school report. A copy of the school report form is sent via mail and is also available by logging in to the admitted student web site. Please ask your counselor to send both to our office by July 1, 2010. "</p>
<p>However, as hard as I search the website, I haven't been able to find a link for the school report form. Can anyone link me to it, or direct me to the page? I need this desperately!</p>

<p>have absolutely no idea. it seems they forgot to include in the admit pack. how about lets collectively not send in final report in protest to this?</p>
<p>I called the Undergrad Admissions Office and they emailed me a copy of the final school report form. It’s so weird though that lots of people never received it…</p>
<p>Yeah, I guess it happens though if they have so many students. Lots of people didn’t get their NetID either, and had to called OIT (like me).</p>
