Forgive me if this is a stupid question, but if I'm getting an extra rec from a coach, and I'm applying online through the common app, do I just mail the letter to the college? Or do I give it to my counselor to send it with my transcript? Thanks!

If you're getting an extra rec from your coach, give her/him the forms he/she needs, including a stamped, addressed envelope. Even though you're submitting the app. online, having the rec subitted via snail mail straight from the coach is fine.

Which forms do I give him? The teacher evaluation form? I was thinking that he was just going to write a letter...

I'm not 100% sure, so you might want to call the college and ask, but I think all you have to do is have him write the letter and make sure that it's labeled "Supplementary" so that they know it's not one of the two primary recommendations.

A letter is just fine. No form is needed for recommendations outside of the ones the college requires.

Should I give it to my guidance counselor to send in, or should the coach do it, or should I do it?

My recommendation: just have the coach do it.
