Should I include this information when applying for transfer?

So this year, I will be a undergraduate freshman, and I plan on applying as a transfer to multiple top universities and I do know that all of them will look at my high school transcript. My high school transcript isn’t all the best, I believe I had a cumulative GPA of 3.14 when I graduated. With my high school GPA I had kind of given up hope applying as a transfer because I believed I would never be accepted based solely on that.

However, around two months ago I was referred to a psychiatrist from my physician to test if I had ADD/ADHD, and it turned out I had have a pretty severe form of ADD that has gone untreated. My psychiatrist was surprised that I was able to maintain above a 3.00 for a person living with untreated severe ADD, and so we decided to look into this further with a psychiatric memory test. And… that test showed that I had an innate ability to memorize subject material quickly when undisturbed by the thoughts that ADD constantly creates in my mind. So in short, my psychiatrist concluded that I was able to maintain a 3.00 because during those moments where my ADD was not as active I was able to quickly grasp the material and memorize it.

AND FINALLY, my question is the information above reasonable to put on my transfer application?

Once you have solid evidence that your GPA is improved while following whatever therapeutic options you and your physician have determined to be effective, then it could be appropriate to include a brief statement that discusses the fact that your ADD had not been identified until you were out of high school, and thus your high school record probably is not an accurate representation of your abilities.
