I was recently diagnosed with mono. I've really had it for a month now, and it's been making me drag, fall asleep in class, fall asleep doing homework, and prevented me from studying. I went to school through all of this, not wanting it to affect my grades too much. I managed to pull off pretty good grades, but a little lower than my usual (94 GPA, last marking period I had a 96). Should I include this in a note with my midyear grades? Or will schools think I'm just looking for an excuse? I know I still did pretty well, but there is no doubt it affected my performance in school.

A 94? Thats a 3.9 GPA which is excellent. Stop being so insecure. You know the answer to this question yourself.

If something like this affects your school performance, the applicant should send an explanatory note, in the same format as an essay. If it is severe, it would be good to have the GC become involved. In your case, it doesn't appear to have affected you much. If you send the note, you run the risk of looking obsessive. While all the colleges want high grades, they really hate grade grubbers. Funny, huh?
