SOS! College recommendations!

I'm applying to USC Marshall in time for the December scholarship deadline.
I can't decide who to write my recommendations.

First of all, does anyone know the definitive number of recommendations needed for USC?
I'm having my yearbook advisor writing one because I'm EIC and I've had him for three years.
I need help deciding between my math teacher and my English teacher. Both teacher like me.

Math teacher:
pros: took BC Calc and taking Multivariable Calculus/Linear Algebra
cons: B in BC Calc and A- currently in Multivar, not exactly the best writer (wow, what a total math teacher cliche)

English teacher:
pros: took English 9 Hn and taking AP Lit, top grade in both subjects with her
cons: she's a tough teacher to please even though she likes me more than the class

Can I pick both? Are 3 recommendations too much?
Also, do I still need a counselor recommendation?

I would definitely send in both if you’re confident that they will both write great things about you. I believe that only one recommendation is required and it can be a teacher or a counselor. I sent in like 4 recommendations last year, so feel free to send in as many as you want as long as it’s not ridiculous (5+).

I have another question that I thought I should post here instead of creating a new thread (I am new here… not so familiar)

1) Does the report and letter of recommendation have to be from the same teacher… the site is very confusing with that aspect.
2) The envelop can be a standard size, right? I mean like the ones used for daily letters.
3) What should the top of the envelope read, in addition to the address of the admission office. Do I have to have my name somewhere?
