<p>I am going to send in a copy of my research paper, but I was wondering what needed to be on the top of the pages (Name, school, application number???)...should it be on every page or just the top one?</p>

<p>Page 2 of the Common App supplement describes what you need to do to submit research, though I don't see the specific reference there to name and DOB (though I know I saw it somewhere).</p>
<p>Definitely include your name on every page. DOB is probably to match up in cases of people w/common names. No worries.</p>

<p>No, they don't. However, there is a box on the Common App Supp to check if you are submitting a paper, so that gives them a data point to cross-check. Haven't seen any reports here of missing research papers among EA apps, so I asuume they have an effective method of putting two and two together. (They also confirmed via email that individual EA apps were complete by November 9th -- that tells me they run a pretty smooth ship over there.)</p>
