Supplement Recommendation and Teacher Specific Questions

<p>Hello, I'm applying to some colleges that allow supplement recommenders and I want my mentor from an internship to write but. But according to the common app FAQ, non-teachers should use the common app Evaluation Form and mail or fax it directly to the colleges that accept such letters. But what I don't get is what the recommender should write for "school address" or "subject taught"? Should he simply leave the fields blank? I have repeatedly contacted the common app help but so far they've repeatedly said just use the Evaluation Form and have avoided my question about what those fields should say instead. help please and thank you.</p>
